Submit an NST Admission Application

Request Official Required Transcripts

Request that all academic college transcripts, high school transcripts or GED scores be sent directly to Nashville State. Hand-delivered transcripts will be considered an unofficial.

Required Military Documents - Air Force

Active Duty - Transcript from Community College of the Air Force or DD-295

DD Form 295 - Application for the Evaluation of Learning Experience During Military Service. This form is used by all branches of service and your Education Center can assist you in preparing these forms.

Required Military Documents - Army

Enlisted Soldiers whose Basic Active Services Dates (BASD) fall on or after 1 October 1981

Army/American Council on Education Registry Transcript System request forms can be obtained from your Education Center or from the AARTS site.

Officers, Warrant Officers or Enlisted soldiers whose Basic Service Date is prior to October 1981

DD-295 can be obtained at your battalion and taken to your Education Division for signature. DD Form 295 - Application for the Evaluation of Learning Experience During Military Service. This form is used by all branches of service and your Education Center can assist you in preparing these forms.

Enlisted soldiers and veterans on active duty as of January 1993 whose Basic Pay Entry Dates (BPED) fall on or after 1 October 1981

Required Military Documents - Reserves

Serving in the Reserves

Copy of Enlistment Contract 4-1 or 4-2 or DD Form 2586, Verification of Military Experience and Training (VMET)

Navy and Marine Reserves personnel can also get a SMART transcript

Required Military Documents - Veterans (Retired or Discharged)

DD Form 214 (Member Copy 4) Certificate of Release or Discharge from Active Duty of Service. Should have Characterization of Service on Form.

Retired or Discharged Military. When a person attended as an active duty student they must submit a DD Form 214 once discharged or retired.

If you do not have this form: It may be obtained from the Tennesee Dept. of War Records or the State Veterans Office of your home of record.

To obtain copies of documents such as military course completion certificates, Academic Evaluation Reports (DA Form 1059), and Certificates of Separation from Service (DD Form 214), Veterans may submit request to:

COMPASS Testing

Immunizations

All students entering institutions of higher learning provide proof of two immunizations with the Measles, Mumps and Rubella (MMR) vaccines, which were administered on or after the first birthday The law further states a student will not be allowed to register for classes until this requirement is met.
